Please only help us test this survey and provide feedback if you are the parent of at least one child who has participated in organized youth sports in the last two years and is between the ages of 7 and 17 years old.
This study is being conducted by Eric Post, Darleesa Doss, Lindsey Eberman, and Matthew Rivera from the Department of Applied Medicine and Rehabilitation and the Department of Applied Health Sciences at Indiana State University. This study has been approved by the Institutional Review Board at Indiana State University.
